Forms add the ability to web pages to not only provide the person viewing the document with dynamic information but also to obtain information from the person viewing it, and process that information.
A form is an area that can contain form elements. Form elements are elements that allow the user to enter information (like text fields, textarea fields, drop-down menus, radio buttons, checkboxes, etc.) in a form.
To insert a form we use the <form>...</form> tags. The rest of the form elements must be inserted in between the form tags.
NOTE: The FORM element has no formatting attributes.
A simple form will look like this:

ACTION: The form's action attribute defines the name of the file to send the content to. The file defined in the action attribute usually does something with the received input.
METHOD: GET (default) or POST specifies which HTTP method will be used to send the form's contents to the web server.
Forms have following fields:
Form elements have properties: Text boxes, Password boxes, Checkboxes, Option (Radio) buttons, Submit, Reset, File, Hidden and Image. The properties are specified in the TYPE Attribute of the HTML element <INPUT> </INPUT>.
Text boxes
Used to provide input fields for text, phone numbers, dates, etc.
<INPUT TYPE="TEXT">
Textboxes use the following attributes:
TYPE: text
SIZE: determines the size of the textbox in characters. Default = 20 characters
MAXSIZE: determines the maximum number of characters that the field will accept.
NAME: The name of an input element
VALUE: will display its contents as the default value.

Password
Used accept text which will be password and will not be showing text while entering. Used to allow entry of passwords.
<INPUT TYPE="PASSWORD"> Password boxes use the following attributes:
TYPE: password
SIZE: determines the size of the textbox in characters.
MAXSIZE: determines the maximum size of the password in characters.
NAME: The name of an input element
VALUE: is usually blank.

Used to send data to the CGI application that you don't want the web surfer to see, change or have to enter but is necessary for the application to process the form correctly.

Hidden inputs have the following attributes:
TYPE: hidden
NAME: The name of an input element
VALUE: is usually set to a value. HTML code for HIDDEN:

Checkboxes
Checkboxes are used when you want the user to select one or more options of a limited number of choices.
<INPUT TYPE="CHECKBOX">
Checkbox have the following attributes:
TYPE: checkbox
CHECKED: is blank or CHECKED as the initial status
NAME: is the name of the variable. VALUE: is usually set to a value.

Radio Buttons
Radio Buttons are used when you want the user to select one of a limited number of choices.
<INPUT TYPE="RADIO">
Radio buttons have the following attributes:
TYPE: radio
CHECKED: is blank or CHECKED as the initial status. Only one radio button can be checked.
NAME: is the name of the variable.
VALUE: usually has a set value.
HTML code for RADIO BUTTONS:
